How We Can Help

Physical Therapy
At-home physical therapy sessions help patients understand how increased physical activity and better lifestyle choices can lead to weight loss, lower blood pressure and improved mental health. An exercise program may also decrease future cardiac events and improve mortality.
Occupational Therapy
Moving from one position or place to another (during performance of everyday activities), such. as in-bed mobility, wheelchair mobility, and transfers (e.g., wheelchair, bed, car, shower, tub, toilet, chair, floor). Includes functional ambulation and transportation of objects.
Speech Therapy
By preventing, assessing, diagnosing and treating speech disorders—including language, communication, cognitive and swallowing disorders, speech therapy is an essential component to the home health model of care.
Often times, patients experience an inability to talk and communicate, difficulty swallowing or poor memory and disorientation. To address these issues, speech therapy focuses on the following areas.
